We have several ways for you to take the survey. The survey is available by QR code, online, mail, and the recreation office at DeWitt Town Hall.

Below, there is a QR code available to scan with your smart phone. Please call with any questions. Directions:

1. Open phone camera. Note: you may need to adjust the distance between your camera and the code for your phone to recognize the image.
2. Hover camera over the code. Note: this may take up to 10 seconds.
3. On some phones, a link will pop up, on your screen. Click this link to open the survey.
4. On other phones the survey will open, automatically.

If you would like to fill out a paper copy of the survey; you may stop into the office or request the survey to be mailed to you. Should you request to be mailed the survey; please call or email us your name and address.

Come to our Recreation Forum too!

In addition to a survey, we would also like the opportunity to speak with you directly.

On Thursday, April 13, at DeWitt Town Hall, we will be hosting two recreation forums, so we may also hear your thoughts and ideas, in person.

The first forum will be 11a – 12:30p. The second forum will be in the evening from 6:00p – 7:30p.

During each forum, guests will have one minute to speak. After the minute, we will allow for questions and comments.
