Parking Permits for people with severe disabilities are issued to town residents who meet New York State’s requirements.  Application forms are available at the Town Clerk’s office and can be mailed to residents upon request. 

Please Note: A new application form signed by a medical provider needs to be filled out each time a parking permit is issued or re-issued.

Requirements:

• Resident of DeWitt
• Completion of New York State’s application form
• Certification of need by an authorized medical provider. 
• Drivers' License or non-driver ID card shown to issuing agent, if applicant has one
